Post-Acute Medical Service

CareDocComm is a post-acute care medicine service provided by the Broad Reach Health Community of Care.

Designed to align the needs of the patient, organization, and practitioners, CareDocComm creates a model in which attending physicians and nurse practitioners work together rather than independently to minimize the challenges historically confronted by physicians in the long term care and assisted living environment.

The service provides an enhanced level of support for all parties and promotes clearer communication among families, practitioners, and staff while allowing for more timely response to facility specific needs.  This model has proven to reduce emergent issues which decreases after-hours calls and avoidable hospitalizations.
