Founded in 1986, our legacy of service to the Cape community spans decades. Broad Reach has evolved into a multi-service healthcare organization which includes Liberty Commons Rehabilitation and Skilled Care Center, Broad Reach Outpatient Rehabilitation Clinic, Broad Reach Assisted Living at The Victorian, Broad Reach Hospice. Each facet of the Broad Reach organization is committed to helping you navigate through critical healthcare decisions which confront you and your loved ones.
We have been recognized as a leader in our field by our peers on local, state, and national levels and are the only skilled nursing facility on Cape Cod to be operated under the same leadership for as long as we have. We are family-owned, and Cape Cod is our home.

Dear Mr. Bogdanovich,
I would like to take to take this opportunity to commend you and the Broad Reach Community for the exemplary care and compassion shown to my aunt, Evelyn Dettenborn, during her multi-level care. She began at Liberty Commons for rehab after a stroke. Doing well, she regained strength and was able to progress to Assisted Living at The Victorian. Carolyn and all the staff at The Victorian encouraged her and eventually she was able to return to her much loved home.
After a period of time her health declined and she was admitted back to long-term care at Liberty Commons. There she met yet another wonderful group of concerned and caring staff. I can never thank everyone enough for their compassion and dignity they all showed my aunt. She was always neat and well cared for, her room always clean and tidy. I was always confident that any concerns I might have were always handled efficiently. In addition to all the staff and supportive departments within Liberty Commons, Broad Reach Hospice made her final months more comfortable with a truly supportive staff. They, in turn, offered me any support I needed during my aunts journey.
With all my heart, I wish to thank each and every member of her healthcare team. You should be very proud of your staff for creating such a positive and supportive environment for your residents and their family members. They are all working together as a team to make a huge difference in the lives of our elderly community.
